  • Request must be made at least 30 days prior to testing date.
  • The candidate must provide a signed letter from a health care provider or other appropriate professional on the professional’s letterhead (including title, address, and phone number of professional) that describes the disability, length of time the candidate has been a patient, and requested testing accommodation.
